These three cover pages must accompany most submissions. Workshop and SIG
submissions only require cover pages one and three.
We have text versions of the cover pages below, which you can either print and fill out, or save, edit, and then print. Cover Page One: Introducing your submission by selecting a participation category, providing contact information, and listing related submissions. Cover Page Two: Describing your submission in detail, including the long list of matching phrases. Cover Page Three: Audio, visual and other needs, for planning purposes.
